CMS is modifying Conditions of Participation (CoPs) to require hospitals, including psychiatric hospitals and critical access hospitals, to send electronic event notifications of a patient’s admission, discharge, and/or transfer to another healthcare facility or to another community provider/practitioner.
The purpose of the rule is to support seamless and secure access, exchange, and use of electronic health information. Hospitals are required to have a solution in place by May 1, 2021 in order to support improved care coordination by empowering providers to proactively reach out to their patients to ensure proper follow-up care after a medical emergency.
